You may also have heard that recent studies do not show a clear benefit to supplementing with certain vitamins; but then why has the FDA recently upped the recommendations for a multi-vitamin, folic acid, and vitamin D for all Americans? To date, research has only scratched the surface regarding the usefulness of supplements.
There are two approaches to supplementation. The first is that if you eat plenty of fresh vegetables and fruits, they provide more of every compound than your genes can use. |

Needed supplements:
• Folic acid: supplementing with up to 1 gram (800 meg) of folic acid daily should help prevent folic acid deficiency.
• Vitamin B6: 50 mg daily.
• Vitamin Bi2: 1,000 meg daily.
Certainly, we're all grateful for medicines when we really need them, despite the reality of risks and side effects. That's why supplementing with fish oils is of huge benefit not only for heart health but also for depression, arthritis, and weight loss.8,1011 However, be aware that the content of EPA and DHA, the active ingredients in fish oil, vary widely in different brands. Look for fish oil capsules and bottled oil that is "molecularly distilled," meaning that the heavy metals that concentrate in fish fat (remember, you're what the thing you ate ate) have been removed. And stop wrinkling your nose! Modern fish oil, particularly Carlson's Norwegian Fish Oil, has absolutely no "fishy" taste. |

For these people, supplementing with extra nutrients, as recommended later in this chapter, will help, as will acid-lowering drugs in some cases. For these people, medication to lower acid may be a necessary part of healing or prevention of ulcers.
The most common treatments for heartburn and GERD are drugs that either 1) neutralize acids, as with OTC antacid drugs like Turns; or 2) reduce the action of the stomach's acid-producing machinery. An excellent book on this topic is Dr. Sherry Rogers' Afc> More Heartburn (Kensington, 2000).

As you've learned, plant compounds have tremendous impact on our cellular functioning, but today we consume fewer plant micronutrients than our ancestors did, making supplementing our diet with vitamins, minerals, and other micronutrients a form of health insurance. What about the government's recommended daily allowances (RDA) for certain vitamins and minerals? Don't I get everything I need in one bowl of Total cereal? RDAs were established after the introduction of refined white flour led to the discovery of vitamin and mineral deficiency diseases. |
| The consequence of this enzyme blockade may take up to a year to develop, but if you have muscle pains or weakness or have developed congestive heart failure, consider supplementing with a daily dose of C0Q10. How Should You Start supplementing?
We recommend that you start with the basics—namely, multivitamin and multimineral supplements—to reduce your risk of having nutrient deficiencies. You should also have clear objectives when you move beyond multivitamins and multiminerals. For example, one might be to reduce your blood-sugar levels. Finally, remember that supplements work best if they accompany a healthy diet and are not used as an antidote for eating too many junk foods. |

You'll recall that sugar increases serotonin levels, making us temporarily less depressed; conversely, dieters get grouchy because their serotonin levels fall. When supplementing with St. John's Wort, some of my patients—and I-report diminished sugar urges; others didn't notice such an effect. The only side effects I've seen are two cases of a mild skin rash that immediately resolved upon stopping the supplement. Dosage:300 mg three times a day

Magnesium
The American Diabetes Association reports that 25 percent of diabetics are magnesium deficient (although other figures place this much higher), and that supplementing with this mineral can improve blood sugar control. Green vegetables such as spinach are good sources of magnesium, because the center of the chlorophyll molecule (which gives green vegetables their color) contains magnesium. Some legumes (beans and peas), nuts and seeds, and whole, unrefined grains are also good sources of magnesium. |

Some of the speakers were working for as many as twenty-five drug companies. |

Since folic acid is involved in the breakdown of homocysteine, supplementing postmenopausal women with this nutrient results in significant reductions in homocysteine levels.100 This association of homocysteine levels and osteoporosis was recently seen in a study showing that high homocysteine levels were associated with almost twice the risk of nonvertebral osteoporotic fractures in women.101 However, in the same study, there was no association between homocysteine levels and BMD at either the femoral neck or the lumbar spine. |
| Keep in mind that fish oil has anticoagulation effects that may act synergistically with medications like warfarin, and therefore caution should be exercised in supplementing with fish oil in people who are taking these medications.81 Lipid-lowering medication was found to decrease beneficial omega-3 fatty acids and increase pathogenic arachidonic acid after only three months of use, leading the authors to conclude that these medications should be combined with diets low in arachidonic acid and high in omega-3 fatty acid. |
| The key to understanding the important need for supplementing with oils rich in GLA, such as evening primrose oil, is that many of us cannot convert LA to GLA efficiently. Dietary deficiencies, disease conditions, processed oils, trans-fatty acids, heated oils, alcohol, aging, viral infections, and sugar consumption block, slow down, or interfere with the enzyme that catalyzes the conversion of LA to GLA. The result is that virtually all North Americans are deficient in GLA. |

In some of these studies of type 2 diabetes, supplementing the diet with chromium was shown to decrease fasting glucose levels, improve glucose tolerance, lower insulin levels, and decrease total cholesterol and triglyceride levels, while increasing HDL cholesterol levels. Although there are also studies that have not shown chromium to exert much effect in improving glucose tolerance in diabetes, there is no argument that chromium is an important mineral in blood sugar metabolism.

Studies suggest, for example, that aging mice, which have reduced levels of brain polyunsaturated fatty acids, appear to show alterations in neuronal membranes, such that the mice show memory loss, learning disabilities, cognitive alterations, and even decrements in visual acuity, which can be reduced by supplementing the mice with DHA containing fish oil or DHA [217]. Similar findings have also been seen in the rat [218]. It also appears that AD patients exhibit lower amounts of DHA in plasma [219] and brain [220]. |
